I understand by submitting this form that I am responsible for any damages (must be reported to the office) that occur and I will clean up & report any spills during my requested time. Please note that water is the only beverage allowed in the gym. I am also aware that the bleachers should be swept out if they are used by my group (a broom & dustpan is located at the west end of the gym). If I am not going to use the gym during my requested time, I will notify the office at least a day in advance to allow other teams to use it. I understand that during the school year, if the MPES is closed due to inclement weather or a building emergency, this facility is closed and I will have to reschedule this event.
